De estas, 261 corresponden a muertes neonatales, es decir, ocurridas en los primeros 28 d\u00edas de vida. Cerca del 90% de las muertes infantiles en el pa\u00eds ocurre en el primer mes.<\/p>\n<p>Los ni\u00f1os no est\u00e1n muriendo a lo largo de su primer a\u00f1o; est\u00e1n muriendo al inicio de la vida. Este dato obliga a mirar antes del parto: al embarazo y, sobre todo, a la calidad de la atenci\u00f3n prenatal.<\/p>\n<p>El propio protocolo nacional establece que toda embarazada debe recibir al menos ocho controles desde el primer trimestre, con seguimiento continuo del desarrollo fetal.<\/p>\n<p>Pero un buen chequeo prenatal no es una simple consulta. Debe incluir evaluaci\u00f3n cl\u00ednica completa, monitoreo del crecimiento fetal, detecci\u00f3n de riesgos como preeclampsia o infecciones, acceso a laboratorios, sonograf\u00edas oportunas y, crucialmente, capacidad de actuar ante cualquier hallazgo.<\/p>\n<p>Ese es el est\u00e1ndar. Reside en Santo Domingo.<\/figcaption><\/figure>\n<p>Sin embargo, en la pr\u00e1ctica, ese est\u00e1ndar no se cumple de forma consistente. Muchas mujeres no llegan; otras inician tarde sus controles o no los completan.<\/p>\n<p>Pero incluso cuando acuden, los chequeos con frecuencia se reducen a consultas breves, sin estudios suficientes, sin seguimiento adecuado y sin un sistema de referencia que funcione cuando aparece una complicaci\u00f3n.<\/p>\n<p>Aqu\u00ed est\u00e1 el problema de fondo: no es solo cu\u00e1ntas consultas se hacen, sino qu\u00e9 ocurre dentro de ellas.<\/p>\n<p>La persistencia de muertes asociadas a prematuridad, asfixia perinatal y bajo peso al nacer \u2014condiciones en gran medida prevenibles con vigilancia adecuada durante el embarazo\u2014 evidencia fallas en la detecci\u00f3n temprana y en la capacidad de respuesta del sistema.<\/p>\n<p>El Ministerio de Salud P\u00fablica de la Rep\u00fablica Dominicana muestra cifras altas de cobertura en salud, pero sus resultados en mortalidad neonatal desnudan deficiencias.<\/p>\n<p>El sistema logra, en muchos casos, el contacto inicial con la embarazada; en otros, ni siquiera eso, y las recibe directamente al momento del parto. En ambos escenarios, falla en garantizar calidad, continuidad y efectividad en la atenci\u00f3n.<\/p>\n<p>La mortalidad neonatal no comienza en la sala de partos, comienza en chequeos prenatales que no llegan o que no cumplen su cometido. No es destino: es una falla del sistema antes del nacimiento.<\/p>\n<p>La atenci\u00f3n prenatal de calidad no es opcional.
